Presentation Abstract:

Implementing human factor engineering and policies to prevent accidents is not a new concept. Many of the contributing causes and root causes of major incidents in the past 30 years have resulted from poor control of human factors.

Human Factor is a discipline concerned with designing machines, operations, and work environments to match human capabilities, limitations, and needs. The human factors approach aims to improve human reliability, minimize the risk of human error, and reduce process safety risks. The session will cover brief history and foundations of human factors, their areas of application, and real-life examples of industry incidents due to inadequate management of human factors in the process industry.

Presenter Bio:

Fahad Al Hajri is a seasoned EHS professional with 19 years of vast experience in Health, Safety, Environment, and Process Safety. He has multi-dimensional exposure in the process industry, involving different phases of projects, construction, commissioning, and plant operation, during his career at Saudi Aramco, General Electric, and Ma’aden Aluminum. He is currently leading the EHS function with an emphasis on Process Safety Culture Transformation at Ma’aden Phosphate Company, KSA. His core technical expertise includes HumanPerformance, Root Cause Analysis, Management of Change, and Risk Management
